RMX - New Carrier Sign Up Form

Thank you for your interest in Rocky Mountain Express/RMX Global Logistics. To request a new carrier sign-up packet, please complete the required fields listed below.

In order to comply with DOT Regulations, it is necessary that an officer of your company sign the following forms:

Contract Carrier Transportation agreement
W-9 Tax Form

We also need copies of the following:

Completed New Carrier Information Sheet
Operating Authority
Current Insurance Certificate listing general liability, automobile liability, workers compensation, cargo & reefer breakdown (if applicable)
A blank copy of the invoice you will be using to bill us

We cannot load your equipment until we have received all of the above information
